Extrinsic Allergic Alveolitis is Inflammation of the internal part of lungs (alveoli), due to inhalation of irritants like dust fungal spores, bagasse (sugar cane dust), avian protein from pigeons and birds, malting barley particles and maple tree bark’s shredding. This condition mostly occurs in farmers and threshers. Inhalation of certain types of organic dust produces a diffuse immune complex reaction in the walls of alveoli. It is characterized by fever, general weakness, dry cough, breathlessness without wheezing after few hours of inhalation of any of the irritants. In farmers, threshers and bird fanciers there are circulating antibodies of the substances they are continuously exposed to , and when the antigen enters again by inhalation, local immune complexes are formed in the alveoli and local inflammation interferes with normal breathing pattern (pulmonary function)
There is another type of alveolitis which is called fibrosing alveolitis a condition that is secondary to rheumatoid diseases or isolated pulmonary abnormality. It is characterized by thickening of alveolar walls as a result of which progressively exertional breathlessness is seen (which is the most striking symptom) along with a persistent dry cough. This condition tends to progress with passage of time.
